This is Immersive Portals for (Neo)Forge. Here's the Fabric version.

Important Notes and FAQ:

It's NOT recommended to use the versions for MC 1.16.x because it's heavily outdated and incompatible with Forge's dynamic dimension feature.
ALWAYS BACKUP YOUR EXISTING WORLD(S) WHEN ADDING A NEW MOD.
