Due to mods for new enemies, bosses, and improving their intelligence, it's very difficult to handle them with a regular sword and shield, and if you're playing on hardcore, it's almost impossible. So what should you do? The answer is to use technology, and the specific technology you choose depends on your playstyle and creativity.
